Requirements for PRE-SCHOOL

1. PSA Birth Certificate - Original and Photocopy

2. Age requirement​

  • Nursery - 4 years old

  • Kindergarten - 5 years old (until October 30 only)

Grade 1

Requirements for GRADE SCHOOL

1. PSA Birth Certificate - Original or Photocopy 

2. Original Form 138-A or Original Report Card (Kindergarten)

3. Form 137-A (Kindergarten permanent Record)

Requirements for JUNIOR HIGH SCHOOL

Documents needed upon enrollment

1. PSA / NSO - authenticated Birth Certificate (Photocopy only)

2. Grade 6 Report Card (Original and Photocopy)

3. Form 137 (Form 10) upon school's request

4.Certificate of Good Moral Character
